NHS Health Checks
The NHS Health Checks programme is a national programme which aims to help prevent heart disease, stroke, diabetes and kidney disease. Everyone between the ages of 40 and 74, who has not already been diagnosed with one of these conditions are welcome to have a health check to assess their risk of heart disease, stroke, kidney disease and diabetes and will be given support and advice to help them reduce or manage that risk.
Eligible Population
Patients with the following conditions are NOT eligible

Coronary Heart Disease
-
Stroke
-
Diabetes
-
Chronic Kidney Disease (stage 3 to 5)
-
Hypertension
-
Atrial Fibrillation
-
Transient Ischaemic Attack
-
Familial Hypercholesterolaemia
-
Heart Failure
-
Peripheral Arterial Disease
-
Being Prescribed Statins
Also, any patient that has received a Cardiovascular risk assessment in the past 5 years or any patients found to with a high risk of Cardiovascular disease will not be eligible.
