If you change your name and address or telephone number please inform us as soon as possible (see Change of Contact Details). If you move out of the practice area, you will be required to reregister with a more local practice.

How To Register as a Patient

If you live within our Practice Catchment Area and wish to register as a patient at our surgery then we need certain information from you and each member of your family so as to be able to trace your previous medical records.

To register as a patient please call into the surgery. We will provide you with a registration pack including information about the Practice and all the forms required to complete your registration. 

Please bring with you 2 forms of identification (ID), with one being in the form of photographic ID and the second being a proof of address (please contact reception for advice if you cannot provide these documents).

Temporary Residents

The Doctors are happy to see patients who are temporarily resident in the Practice area should medical advice be required. In this instance persons wishing to be treated should state that they are a temporary resident when reporting to reception.

To register you will need to bring your passport and complete our Temporary Resident Registration form which is also available from reception.

Named Accountable GP for all Patients

A requirement in the GP contract is to allocate ALL patients with a 'named, accountable GP'.

As stated on the Britsh Medical Association (BMA) website:

The contract requires the named accountable GP to take responsibility for the co-ordination of all appropriate services required under the contract and ensure they are delivered to each of their patients where required (based on the clinical judgement of the named accountable GP).

Please note that patients are still free to see ANY GP of their choice. Patients will start to be notified of their accountable GP when they attend the surgery, if you are unsure who your accountable GP is and would like to know please contact the surgery on the main telephone line.
